Everything else gets discussed eventually, and health insurance seems to be a topic of heated discourse in one of the threads, so here goes....

What options exist for small business owners and employees in shops with well less than a dozen employees including the shop owners?

Let's assume it's a small shop, could be restaurant, key maker/locksmith, tool grinder, t-shirt silkscreener, photographer, bicycle wheel builder, you get the idea - small shop, no large scale supplier to tag onto the policy of. Looking at basic medical, major medical, short term and long term disability options, dental, prescription service, and so forth.

Just looking to collect data and leads and general not too specific info at this point. I might be going independant in the next 6 months or less to three years or more. Plus I know a few other folks who are also in the medical insurance "about to loose it" zone who are also looking at any options available apart from dressing down and trying to wander into various free clinics for assistence.

Thank you for any freely shared advice or insights or leads or concrete real world data. I know it's expensive, but I have heard of consortiums of sort set up for small businesses to link insurance needs together and save costs by combining volume.
