Ann's update tonight:

So we made it through Thursday.
The trial went better this morning than yesterday, but he still gets too agitated to remove the tube just yet. He has to be stable before they remove it. The things they are looking at are his heart rate, blood pressure, ability to comprehend what is being said to him and how much he is sweating. His heart rate was lower today as he came out of the sedation, and he did seem to comprehend a bit more. Since he was doing a bit better with the morning trial, the doctor put him on a transition sedation. It is a combination sedation/pain medication which will have him not quite so heavily sedated. They had to gradually turn down the heavy sedation before starting the transition medication.
Once the heavy stuff was low enough, we did another trial. He did even better this afternoon with his agitation level. Me, his nurse and respiratory therapist were able to talk him down and calm him down quite a bit more than the previous days. He was able to respond to commands (squeeze my hand, take slow and deep breaths), his heart rate was again not so elevated, but he's still sweating profusely and having bouts of confusion and panic. They began the new combo drug and he drifted back off to sleep. We'll try again tomorrow.
Dr. Hartman, his neurosurgeon, removed the drain from his head, said he was very pleased with how his incision looks and with his recovery thus far. He had hoped that he would be doing therapy by now, but he also said the brain can't be rushed and will have to heal at its own pace. The big hurdle to get over right now is getting that tube out so they can better assess neurological function and to see if he has regained the ability to swallow so they can start him eating by mouth. If he is still having trouble with swallowing, they will have to move his feeding tube directly to his stomach, which he said is a very easy procedure. A feeding tube usually goes through the nose, but given all the work he did on his sinuses, he cannot have it that way.
His nurses and the ICU director ganged up on me last night and convinced me that I need to be coming home at night and only be there during the day. His day nurse that he had yesterday, Rochelle, went and got the day nurse he had last week, Tammy, to come and talk to me. Tammy came and asked me if I was eating and sleeping, and that I'd better not lie to her. I told her I was. And then she told me not to be mad at Rochelle, that she was a good Christian woman, and that she was just worried about me, especially after the trials. Tammy reminded me that nothing happens during the night - that the doctors and procedures are all done during the day and that's when I needed to be there. Even if I come home and don't sleep - just lay in my own bed all night - I'd be more well rested than sleeping on that couch in his room waking up every time someone came into the room. She also told Rochelle "She's not an unreasonable woman, she's just anxious about her husband." Rochelle then explained to me that she was the one that had "snitched on me" because she saw how the last couple of days had taken their toll on me. She then told me that he knows when I'm there - he can sense it. When she goes to check on him, his blood pressure is lower when I'm in the room than it is when I'm not. I do periodically leave the room just to go get coffee, or fresh air, or go eat. But, she said he can also sense when I'm upset, and that can have a negative impact on him. The ICU director came and went over much of the same information about how I need to take care of myself to be able to care of him. Then his night nurse, Pam came on for her shift. She was the one that was with me last Wednesday when he was having the seizures so bad. She said "Honey! You're still here! Why are you here? You shouldn't be here!" I told her it was ok, because I had been evicted. Lol! She then told me that she'd take good care of him and for me to go on home.
So I did. And I'm home tonight. And I guess I'll continue on this routine until he leaves ICU, or he progresses to the point that I can be of more good.
I'm really not an unreasonable woman. I just miss him. We all miss him. All of "him" is here at our house, but he's not here. And not knowing when he's coming back, makes it even harder to be here without him. It's not about me right now - it's all about good positive energy flowing in his room so he can continue to improve and get back where he belongs.
As always, love and thanks to everyone!

Do not always trust hospital staff to have the patient's best interests at heart. People get lazy and jaded. Few are those who do not. It's human nature. Hospitals are businesses.

Always ask every nurse and doctor involved what the worst case scenario is for the patient, and what should be done to preclude it. You'll usually get different answers that will help you help them be more proactive.

Multiple nurses have told me that patients receive better care when loved ones are there at the hospital and attentive to the patient.

Please tell Ann to do what she feels is best and not to let hospital staff dissuade her. They are not looking out for her or Dave's best interests.

I've seen two people die because of hospital staff laziness and/or optimism (failing to address the worst case possibility).

Lord knows I spent countless hours in hospitals with my parents prior to their passing and there is no more wearying place to be. Of course it doesn't help that you have entire families (including obnoxious kids) that like to camp out 24/7 in hospital waiting rooms denying you any place to get some peace when you're not in the room with your family member.

In my experience, when someone's in an ICU, they get VERY good care and there's not much you can do as a family member to help. Once they get out of ICU, I'm 100% in concurrence with what Blake's heard from nurses. Some nurses are just slack and many are overloaded. I honestly can't imagine how some people survive any kind of extended hospital stay if they don't have family there to look out for them.

Hopefully Ann's getting help from local friends/family.

My mom has been in and out of the hospital for the last 6 weeks... Some kind of liver distress that caused extremely bad jaundice. There is also a pancreatic tumor that is currently operable except for one thing... All the time that has past has made her so weak that she won't survive the surgery. Insurance is trying to kick her back out again to come home. And the long delays from onset to where she is now are mostly from all the "let's wait and see if this did anything" while she grew weaker and weaker.

Why I say all this is to back up what Blake said.

Also my advice is to gather every single medical record and keep it in a safe place. Every test, every report, every everything! You can not believe how many times my mom's records have been lost and we had to supply fresh copies. This mostly happens when moving from one facility to another, like when Glitch gets moved to a longer term rehab facility.

LONG update from Ann this morning. Progress continues:

quote:

Good morning all! I just wanted to drop a quick update to let everyone of you know that your prayers, good mojo, thoughts, and positive energy thrown out into the universe are working!

Dave has been out of the "coma" for, let's say 1 1/2 days. I don't really count Friday, because he was still in and out of conciousness, I'm sure from just letting all the meds get out of his system. He was confused and agitated, especially when he realized the right side of his body was not working. He still couldn't talk, but they said to give that a couple of days to allow his vocal cords and throat to rest from having the breathing tube.

Yesterday was his first full day awake. He's responsive and seems to understand most of what is being said to him. When he gets tired, or there are too many people talking at once, he tends to get confused. He can now nod and shake his head, which hallelujah!, he wasn't doing on the Monday we took him to the ER or since. It was a very emotional day for him, as he could more comprehend what I was telling him about the events that have taken place over the last 2 weeks. There were times I had to back off, because it was just too much too soon for him to process.

We've received cards and letters, which I had taken with me to the hospital and I read to him. I also got on my laptop and read him some of the comments from Badweb and Facebook. He waved his hand at me to stop, because it was just too much for him to take just yet. He's just as touched as me and the boys at the incredible outpouring of love and support.

Friday, after the tube was out and he was out from under the sedation well, his Respiratory Therapist came to check on him before his shift ended. Alfonso was one of the first members of his care team when we arrived in ICU. He was the one that had intabated him that wild Thursday morning. He was also the one that removed it. I asked Dave if he remembered Alfonso and he looked at him kind of like he did. Alfonso said "I'm your respiratory therapist and I removed your tube earlier. i just wanted to check on you before I left for the day." Dave lifted his left hand to him and grabbed his hand in and pulled him down close to him for a bro hug. It was as if he was saying "Alfonso, you are my BEST friend today!" Several of the nurses he had last week came to visit him once the tube was out and he was awake, telling them how happy they were that he's doing better. The PT did an assessment yesterday, and said he is much stronger than he was before the surgery. He will have his first full PT session today, and she hopes to have him trying to walk tomorrow. She said he needs to be out of the bed for most of the day sitting in a chair and she wants him to eat at least 2 out 3 meals a day in the chair. The speech therapist also came and assessed him, and said even though there is still a delay with his swallowing, she felt confident that he could handle a puree diet for now. Needless to say, dinner time was not fun. I mean, honestly, pureed chicken? Blech! He can't talk, but he let me know in no uncertain terms was he eating THAT. She also said he's much better than he was before the surgery.

The strength and determination in this man touches my heart in a way I cannot express with words. He kept me awake most of the night trying his best to talk. Even after I laid down on the couch in his room, he was still making sounds and trying so hard to make at least one word. He's begun his own PT, lifting his right arm with his left and moving it around. He continually raises it trying to grab on to the bed railing, forcing it to do what it does not want to do. At one point when I was at his bedside talking to him, he grabbed my hand with his left and had me hold my hand up in the air (like a high five) and he would try to touch my hand with his right hand.

He's frustrated and agitated, but he realizes it's going to take time and patience to come back 100%. We were told that speech would be the last thing to return, but he's still able to "communicate". I have to remember not to ask 2 part questions so he can answer yes or no. We're trying to figure out our new temporary normal so that we're not just staring at each other all day. He does seem to "connect the dots", laughing where he's supposed to, smiling at the nurses when they talk to him and follow the commands of what is asked of him. This means his brain is working at near 100%, it's just got to remember how to talk to the rest of his body.

I'm still keeping visitors to a minimum to give him time to rest as much as possible during the day. He's going to need all his energy for the therapy sessions over the next several days. As I said, when he gets tired, he gets confused and agitated. His focus now needs to be on getting that brain to tell that body what it needs to do so he can recover enough to come home.

The boys came to visit him last night and it was pretty emotional for the 4 of us to say the least. Our king has awakened!

I read somewhere once that "one voice is but a whisper; several voices will soon roar". I think it had something to do with speaking against the government, but I think it holds a lot of water in this situation. God, the universe, all the deitys, and even the Spaghetti Monster have heard the immense roars of friends and loved ones and total strangers begging and demanding at times for healing, hope, power, peace and strength to this man of mine. And for that, this family will be eternally grateful.

Please continue the prayers. It's still going to be a long road, but it's off to an incredible start.

I'm not much for going to church, but I look for God's grace and spirit everywhere I can in this world... sometimes it's hard to find.

Today's not one of those times. I can't think of much of anything to improve this Sunday morning than the news I just read.

Blessed be, Glitch and Ann! So glad to hear of this great progress!
